In this episode of Tooth or Dare, Joe Siegfried and I unpack the current climate of dental hygiene in Canada and challenge the idea that fewer hygienists are entering the profession.

Graduation numbers and class sizes haven’t declined. What has changed is retention and migration.

🇨🇦 Canada has approximately 33,600 registered dental hygienists, yet distribution varies widely ▪️ Rural and remote communities continue to struggle with retention ▪️ About 9% of hygienists plan to leave the profession within 2 years, and 18% within 3–5 years ▪️ Temping is at an all-time high, reflecting burnout, flexibility needs, and shifting work preferences

As the profession evolves, traditional 9–5 practice models are being challenged. The workforce exists, but it’s moving differently than it used to. Joe brings important nuance to a topic that’s often oversimplified.

We also explore a deeper shift happening across dentistry: how clinicians define value.

Value in dental hygiene isn’t always about money or production. And practice isn’t limited to the box you work in.

Joe and I discuss how value shows up through discussion, treatment planning, culture, ideas, flexibility, autonomy, respect, growth, and alignment, not just a few extra dollars of production. When the definition of value shifts, so does how, where, and why people choose to practice.

▪️ How $99 dental promotions can mislead and limit patient access ▪️ Why these deals may not be as good as they sound ▪️ How patient trust is affected by unclear pricing ▪️ Stories from people who felt let down by the experience ▪️ The bigger impact discount dentistry has on the profession

Some dental offices advertise low-fee cleanings to attract patients, especially those without insurance. But when you call to book, the information often changes. What sounds legal on paper can cross the line when the details aren’t disclosed up front.

The Royal College of Dental Surgeons of Ontario (RCDSO) requires that all advertising and promotions be:

• Accurate and Verifiable: Fees, services, and any conditions must be truthful and backed by facts.

• Transparent on Limitations: Any restrictions—such as “new patients only,” extra fees for exams/X-rays, or limited-time offers—must be clearly stated in the ad itself, not revealed only after a patient calls.

• Non-Misleading for the Uninsured: Promotions targeting patients without insurance cannot imply that essential care will be fully covered if additional treatment is required.

From what I've read, many patients have left negative reviews after feeling bait-and-switched, and there are repeated reports of high staff turnover in these offices, with employees leaving reviews under alias names. This is another warning sign that the culture behind the marketing may be just as shaky as the promotion itself.

If you care about honesty in healthcare and want to know what really happens behind bargain-basement dental ads, this episode is worth your time. Perhaps motivation for those to stop subscribing to marketing like this, and I hope it finds the right ears better to regulate this shady form of marketing in dentistry once and for all—especially for those of us doing it cleanly.

"Irene, If things continue, you'll have vision loss."

"I thought I was here to get a cleaning... I mean, new glasses!" 

After a routine eye exam, I left with more than just a new prescription; I walked out with a moment of clarity and panic.

While I was distracted wondering how much my insurance would cover for new sunglasses, the optometrist said something that stopped me cold:

"Your eyes are 200% worse this year than they were last year."

It wasn’t vague. It wasn’t sugar-coated. It was direct, backed by a scan, and paired with a clear treatment plan. If I didn’t follow through with the exercises, medical devices, and recommendations, I could face long-term vision loss. Simple. Clear. Urgent.

And it hit me: this is exactly what patients need from us in the dental chair, clarity without judgment, and a transparent, actionable path forward.

In this deeply personal episode, I share the whole story behind my recent academic appeal to George Brown College after being declined admission to the S305 Honours Bachelor of Science in Dental Hygiene Degree Completion program in October 2024. The original reasoning was that 21-year-old Irene went to the "wrong" school 17 years ago. One that didn't prepare 39-year-old Irene for the academic journey of an online degree completion program.

This episode covers every step of the journey, from the initial application process to receiving the decision and ultimately crafting my formal appeal. I also read the full appeal letter and reflected on what it means to advocate for yourself, especially in an academic setting. Whether you're a dental professional considering a degree completion program or someone who’s ever felt the weight of rejection and the courage it takes to respond, this episode is for you.

Let’s discuss the complexities of higher education, professional growth, and the sometimes nonlinear paths we must take.

Tony is a double-degree Oral and Maxillofacial Surgeon. He received his DDS and MS in Anatomy and Cell Biology from Indiana and his MD and internship/residency from Vanderbilt. While at Vanderbilt, he held a research fellowship with the National Institutes of Dental and Craniofacial Research, focusing on laser-induced intrauterine surgery and creating animal models with facial growth anomalies. 

He has been in private practice in Nashville for the past 46 years, focusing on the most complex types of maxillofacial surgery.  

During his long career he founded charity medical clinics in both Haiti and Jamaica  and is proud to have 54 years as a private pilot prompting many “hold my beer” moments. 

Based on 10 years of research, he discovered the common denominator for TMJ/TMD and is now the president and CEO of TMJ Services which has developed  a network of doctors dedicated to the non-surgical solution for this devastating and very common disease. 

Tony’s greatest joy in life is spending time with his three grown children, six grandchildren, one great grandchild and his wife Ann.

Meet Dr. Amanda Wilson, a San Francisco native whose journey in dentistry has taken her from the heart of the Bay Area to the beautiful shores of Honolulu, Hawaii. Dr. Wilson earned her dental degree from UCSF Dental School and went on to achieve her Master of Dental Science and Orthodontics Certification at the University of Connecticut. With over a decade of clinical experience, she has completed thousands of orthodontic cases, both fixed and removable.

Driven by a passion for teaching and mentoring, Dr. Wilson transitioned into corporate dentistry, where she has spent the past few years as a Dental Director and Consultant for both national and international orthodontic companies. Her work focuses on innovative areas such as early treatment, functional appliances, aligners, and tele-dentistry.

In 2016, Dr. Wilson saw a gap in orthodontic coaching services tailored for General Dentists and took the initiative to fill that need. She founded her own orthodontic management and coaching company, StraightSmile Solutions®, to provide guidance and support to her peers.

Now living in Honolulu with her family, Dr. Amanda enjoys the vibrant local culture and outdoor adventures. When she’s not working with clients around the globe, you can find her surfing, hiking, or leading her Cadette Girl Scout troop. She’s also a proud USA Swimming Official, further reflecting her commitment to community and youth development.

Dr. Amanda Wilson made big waves in Hawaii’s orthodontic world, not just in the clinic but also in the law! Back in 2005, she helped push for a bill (Act 121) that temporarily allowed orthodontists and other dental specialists to skip the usual state dental exam and get licensed based on their credentials. This was a big deal because at the time, Hawaii was changing its licensing exam and didn’t have a clear process in place.

• Dr. Wilson lobbied for and helped shape the bill because she saw a gap in the system—qualified orthodontists couldn’t practice in Hawaii due to outdated licensing rules.
• Act 121 created a temporary shortcut for dental specialists to get licensed if they had the right education and experience.

• In 2005, Dr. Wilson applied for her own Hawaii dental license using this new law.
• But the Hawaii Board of Dental Examiners denied her application because they said her graduate training (residency) didn’t count as actual "practice" under the law.
• Even though she had worked independently in California for 10 months, Hawaii required three full years of practice outside of school.

• She took the Board to court, arguing:
  • Her residency training was in real-world dentistry and should count.
  • She helped write the law, so it should have included her situation.
  • The Board’s decision was unfair and possibly biased against her.
• The court sided with the Board, saying their interpretation of the law made sense.

• Even though Dr. Wilson wasn’t granted her license, her efforts paved the way for discussions about better licensing options for specialists.
• This highlighted flaws in Hawaii’s system—showing how tricky it is for well-trained orthodontists to get licensed, even when they’re fully qualified.
• While the law was short-lived (it was automatically repealed when the new licensing exam was introduced in late 2005), it gave other specialists a brief opportunity to practice in Hawaii without unnecessary barriers.

In 2016, Dr. Wilson saw a gap in orthodontic coaching services tailored for General Dentists and took the initiative to fill that need. She founded her own orthodontic management and coaching company, StraightSmile Solutions®, to provide guidance and support to her peers.

As a cosmetic dentist, Dr. Andi actively works to change the public perception of dentistry and make it about looking good and feeling great. Dr. Andi has a strong commitment to teaching and frequently travels to lecture on cosmetic dentistry.

She also holds several academic roles: Assistant Clinical Professor at NYU’s Aesthetic Dentistry Department, Clinical Professor at Nova Southeastern University, Residency Masterclass Instructor at STRIBE, Co-Founder and Clinical Instructor for the Art of Ceramic Hands-on Masterclass, and Clinical Instructor and Lecturer for the Rosenthal Institute’s Hands-on Aesthetic Advantage program in New York.

In Part 2 of this conversation with Dr. Luchtefeld, we learn about how he came to be President of the American Equilibration Society (AES), a leading organization of dental professionals that has been around since 1955. Dr. Jason Luchtefeld, DMD A 2000 graduate of Southern Illinois University School of Dental Medicine, he was an active leader in the ASDA, AADS (now ADEA) and the AGD.  Following dental school, he completed a General Practice Residency at the VA Hospital in Denver, CO.  His technical training has encompassed learning from the Dawson Academy, the Misch Institute, R.L. Frazer & Associates and many other respected learning organizations. 

In 2005 Dr. Luchtefeld obtained his Fellowship in the AGD (Academy of General Dentistry ) and the ICOI (International College of Oral Implantologists).  In 2007 he joined the AES – Occlusion, TMD, Comprehensive Care (formerly American Equilibration Society) and quickly rose to leadership.  He first served three years as AES Exhibit Committee Chair and a member of the Strategic Planning Committee (SPC).  After a year on the SPC he Co-Chaired the most successful and transformative AES Strategic Planning Process in more than 55 years.  During the Applied Strategic Planning process (led by Dr. Bob Frazer) a passion was ignited for the power of the process.  This passion has continued in Jason overseeing implementation and ongoing monitoring of the AES ASP – allowing the ASP to become the central governing document for the society.  

In 2019, Jason began expressing his passion and purpose for asking questions into the formal process of Applied Strategic Planning as a partner in Inspero – a strategic planning and coaching firm based in Austin, TX.  His goal is to help dentists and their organizations question, create, and then inspire their best futures.  Jason’s experience in this arena has rapidly grown to include co-facilitating several ASP’s for dentist’s and a corporation as well as leading the process for several dental organizations.  He has also led the Organizational Health Process through multiple iterations.

In 2024, Jason joined forces with Dr. Kevin Kwiecien to form The CCO Solution to provide fractional chief clinical officer functions to growing DSO’s.

Balance.  She received her Bachelor of Science degree and earned her DDS degree from the University of Toronto. Upon graduation, she completed a one-year General Practice Residency at Mount Sinai Hospital in Toronto, ON. Dr. Habsha received her Diploma in Prosthodontics and Master of Science degree, both from the University of Toronto. She is a Fellow of the Royal College of Dentists of Canada (RCDC) and is an examiner and past Section Head for the Oral Examination in Prosthodontics for the RCDC. Dr. Habsha is an Adjunct Assistant Professor at the Department of Dentistry, Eastman Institute for Oral Health at the University of Rochester Medical Center. She has served as an Assistant Professor at the University of Toronto and currently instructs at the graduate level in Prosthodontics at U of T. Dr. Habsha is a Professor at George Brown College of Applied Arts and Technology and is the On-staff Prosthodontist at MedCan clinic in Toronto. She holds an appointment as Staff Prosthodontist at Mount Sinai Hospital where she instructs the dental residents and is involved in various clinical research projects.

Dr. Habsha is an Associate Fellow of the Academy of Prosthodontics and Greater New York Academy of Prosthodontics, a Fellow of The Pierre Fauchard Academy and holds memberships in numerous Prosthodontic organizations and societies. She is a trailblazer and founder of Women in Dentistry: Work.Life.Balance, an organization dedicated to educating, networking and empowering women in the dental field. Starting a dental business from scratch obviously requires a lot of money, time and effort - and then you need patients to actually come to you. But what if you could bring dentistry to your patients instead? That’s exactly what Dr. Vorholt decided to do, while still coming home every night to his family. Tune in to hear how he maintains work and life responsibilities using his experiences running a private practice, and reflections on lessons learned from a failed business partnership. 

Dr. Steven Vorholt: Dr. Vorholt was born in Cincinnati, Ohio and later moved to Columbus, Ohio to attend college. He graduated with a Doctorate of Dental Surgery from The Ohio State University in 2013.

Shortly after graduation, Dr. Vorholt started a private practice in the Columbus area. Immersing himself in as much continuing education as possible, he eventually developed a great passion for dental implant surgery. In 2019 after 6 years of building a successful general practice in Ohio, he followed his passion and moved to Scottsdale, Arizona to take a full time position focusing solely on implant placement and restorations and also worked in dental education. He taught courses such as Basic Implantology, Sinus Grafting, Full Arch Overdentures, Implant Complications, 3D Printing Surgical Guides and more. Dr. Vorholt also helped over 1,000 dentists place over 12,000 implants in his time in live surgery education.

In 2021 Dr. Vorholt received his Diplomate in the American Board of Oral Implantology. In 2022 he became a Fellow in the American Academy of Implant Dentistry and was asked to present at the AAID Annual Conference for two different workshops and lectures. He lectures nationally on all topics of dental implants and loves to partake in CE from other great educators whenever given the chance.

What do you do after your business partnership falls apart? According to Dr. Vorholt, continue to invest in yourself and come out stronger. Like many new dentists, he graduated with heavy student debt, but has never let that get in the way of starting a practice and living life the way he wants. Even when things ended with his partner, he switched gears into education, continuously learning and giving back to his peers, teaching other dentists about implant surgery. Dr. Steven Vorholt: Dr. Vorholt was born in Cincinnati, Ohio and later moved to Columbus, Ohio to attend college. He graduated with a Doctorate of Dental Surgery from The Ohio State University in 2013. Shortly after graduation, Dr. Vorholt started a private practice in the Columbus area. Immersing himself in as much continuing education as possible, he eventually developed a great passion for dental implant surgery. In 2019 after 6 years of building a successful general practice in Ohio, he followed his passion and moved to Scottsdale, Arizona to take a full time position focusing solely on implant placement and restorations and also worked in dental education. He taught courses such as Basic Implantology, Sinus Grafting, Full Arch Overdentures, Implant Complications, 3D Printing Surgical Guides and more. Dr. Vorholt also helped over 1,000 dentists place over 12,000 implants in his time in live surgery education.

In 2021 Dr. Vorholt received his Diplomate in the American Board of Oral Implantology. In 2022 he became a Fellow in the American Academy of Implant Dentistry and was asked to present at the AAID Annual Conference for two different workshops and lectures. He lectures nationally on all topics of dental implants and loves to partake in CE from other great educators whenever given the chance.

If you’ve ever felt the struggle of balancing your work, health, and relationships with loved ones, you’re not alone. David shares lessons he learned going through the challenges life threw at him.

Welcome to the 146th episode of the Tooth or Dare Podcast with Irene Iancu (@toothlife.irene) and this week's guest Dawn Winkelmann (@msdawnslp).

Do you remember learning how to eat? Were you a picky eater growing up? What does this have to do with dentistry? Surprisingly, a lot - keep listening to find out! 

Infancy is a critical stage of development, a time when babies learn survival skills and good (or bad) habits. Dawn emphasizes the importance of baby-led mealtime, in which babies learn to feed themselves instead of being literally spoon-fed by parents. Eating is a sensory experience with so much going on, and it’s crucial for parents to make sure their child is swallowing correctly, getting enough nutrition, and exposed to a variety of foods to prevent potential issues down the road. Having the right knowledge is not enough however, so Dawn also shows us how her specially designed baby-led utensils help teach tiny humans the safe way to enjoy liquid and solid foods.

Guest Bio:

Dawn Winkelmann is a Speech-Language Pathologist and Pediatric Feeding Therapist specializing in infant swallowing. She has over 25 years of experience teaching parents, therapists, and caregivers how to feed children of all abilities. Through her private practice, Ms. Dawn provides in-home feeding therapy and online coaching, helping parents start solids safely with the baby-led weaning (BLW) approach. She also teaches techniques to help with picky eating, tube feeding, children with special needs (Autism, Down syndrome, etc.), as well as medically compromised kiddos who struggle to eat a variety of foods + liquids.

She has dreamed of being a dentist since she was a little girl growing up in the dentist's office where her mother worked as a dental hygienist. Dr. Britany is passionate about forming personal relationships with her patients and working together to create a treatment plan to meet each patient's unique goals for both an aesthetically pleasing and healthy smile. She completed her undergraduate education at Eastern Kentucky University and received her DMD from the University of Kentucky College of Dentistry.

Dr. Britany is originally from Hazard, Kentucky located in the foothills of the Appalachian Mountains. Coming from an impoverished area has made Dr. Britany passionate about educating people on the importance of establishing healthy oral habits starting from a young age. She has spent time volunteering in low-income clinics treating adults and children who would not otherwise be able to afford care.

Dr. Britany lives in Charlotte with her beagle, Ryder, and Jazz the cat. In her free time, she enjoys hiking, rock climbing, reading, cooking, and yoga. She is a soccer fan and cheers on Tottenham Hotspurs.

Ben Johnson was inspired to go into oral surgery by his father, Dr. Carl Johnson, and their lifelong friend and colleague, Dr. Rick Edwards. He saw the differences they were able to make in patients’ lives and wanted to be able to do the same. Now, Dr. Ben works with his father on a daily basis and feels extremely lucky for the opportunity. He shares a similar philosophy with the other oral surgeons at Pacific Northwest Oral & Maxillofacial Surgeons regarding patient care and satisfaction, which centers on providing highly focused, patient-centered treatment.

Dr. Ben strives to treat every patient like they are a part of the family. He understands that coming into the office for oral surgery can be a potentially anxious process, and he works hard to make each patient’s experience as comfortable as possible. Dr. Ben enjoys practicing oral surgery because it provides him with the opportunity to improve patients’ lives.

In this conversation with Dr. Ben Johnson, we chat about the profession of oral surgery, where he went to school, where he grew up and how Ben got to where he is today.

We recorded this episode in Boston live on the tradeshow floor at Yankee Dental Congress and we had a jam-packed conversation about the future of dentistry and diagnostics with both Katrina and Robert that is new to Irene but will be implemented into her therapy in 2023 which is exciting for her and her team.

In this podcast episode, Katrina shares an incredible story of how she got engaged this past year and opened a wine brand called the Wine Op. Both happen simultaneously and take a considerable amount of time and energy. 

Behind her back, her Lumber boyfriend (you've heard of him a lot by now) is secretly planning an extra special wine launch party that comes with a not-so-little accessory.  Katrina had no clue despite all the signs pointing to it. Classic Katrina for managing to be completely oblivious.

Furthermore, Irene reveals that she's on track to have a Christmas tree for the first time in five years and explains why she needs to be more conscientious of her personal life this year. Since the last episode, she's also made some life and career changes, including attending the Glo Good/Twice/Let Love Rule mission trip in the Bahamas we encourage you to check out and perhaps consider supporting. The real shocker hits at the end, when Katrina announces her intention to leave the podcast - which was something both ladies had known for a while but didn't want to share just yet. Despite their divorce from podcasting, they will remain friends, so don't worry; you'll see them at a hotel lobby bar comparing slide decks at the Star of the North, where both ladies will be dropping knowledge live on stage.
