Drawing from real‑world experience, this conversation explores the psychology behind resistance, the limits of “just educating,” and the communication mistakes clinicians make without realizing it. Most importantly, we dig into what actually drives behavior change and how providers can shift their approach to create trust, connection, and meaningful follow‑through. If you’ve ever wondered why your best advice doesn’t stick, this episode offers clarity, insight, and a practical perspective.

In this episode, I’m diving into HDF (hemodiafiltration) machines — a therapy that has been widely used around the world for years and is now becoming increasingly available here in the United States. With HDF finally entering our dialysis landscape, it’s time for dietitians, nurses, nephrologists, and all dialysis clinicians to understand what makes HDF different, why it matters, and how its use may impact the patients we care for every day.

I want to be clear: I’m not claiming to be an expert. I’m learning about HDF in real time — the same way many of us are — through my own research, reading, curiosity, and conversations within the field. My goal is to explore the topic openly, ask the right questions, and help all of us build a working understanding of how HDF may change clinical practice, nutrition management, fluid strategies, and quality metrics.

HDF is here, and it’s coming quickly. The more we understand it, the more prepared we’ll be to support our patients and collaborate effectively across the care team.

In this episode, I break down the historic restructuring of the U.S. organ‑transplant system as UNOS’ decades‑long monopoly is dismantled. You’ll get a fast, clear overview of why the federal government is splitting up the roles of the United Network for Organ Sharing, how the new multi‑vendor OPTN framework will work, and what this shift means for the people at the center of transplantation: clinicians, patients, and transplant centers.

I cover the motivations behind the change—technology failures, inequities in organ distribution, and longstanding concerns about transparency—and explore how a more competitive, modernized system could improve allocation, reduce waitlist deaths, and increase accountability. Finally, we look at the practical ripple effects: what clinicians should expect operationally, how centers may need to adapt to new vendors and systems, and why patients might finally see a fairer, safer, more reliable organ‑matching process.

A quick, essential briefing on the biggest transplant‑policy overhaul in decades.

In this episode, I break it all down with clarity and confidence. You’ll learn what amyloidosis really is, how it affects the body (you want to know about the kidneys, but included are other organs critical to overall assessment), and why early recognition matters. We’ll explore the different types — from AL to AA to dialysis‑related amyloidosis — and highlight the red flags clinicians should keep an eye out for.

Then we shift into the practical side: how to support patients nutritionally, what to prioritize in counseling, and how to navigate the unique challenges these patients face. You’ll walk away with actionable strategies, communication tips, and a deeper understanding of how to treat these patients so YOU and THEY feel empowered rather than overwhelmed.

<itunes:summary>Amyloidosis can feel intimidating — rare, complex, and packed with nuances that make even seasoned clinicians pause.

Special Urgent Update — UNOS, OPTN Modernization, and What’s Next
A focused, urgent briefing on dramatic changes to the national transplant system and what they mean for patients, clinicians, and advocates. We unpack why the OPTN transition matters now, what the October 2025 pause and contract timeline mean for continuity, which OPTN components have already moved to new contractors, and the biggest risks and near‑term decisions that could affect waitlists, allocation, and patient safety.

Run time: ~6 minutes
• Why listen: Clear, actionable context you can use today—no filler.
• What you’ll learn:
• The federal plan to replace UNOS with multiple vendors and the December 29, 2025 contract deadline
• Which OPTN functions have been awarded (Board support; Patient Services Line) and which remain unassigned (waitlist/allocation software)
• How the October 2025 shutdown paused key OPTN work and why that raises continuity and safety concerns
• Key policy work to watch now, including the multi‑organ allocation proposal

In this episode, we conclude our exploration of the gut axis and microbiome, focusing on its role in kidney disease. We break down how imbalances in gut bacteria—known as dysbiosis—can worsen chronic kidney disease (CKD) by increasing the production of uremic toxins like indoxyl sulfate and p‑cresyl sulfate. These harmful metabolites contribute to inflammation, oxidative stress, and faster disease progression.
At the same time, we highlight the protective role of short-chain fatty acids (SCFAs), produced when beneficial microbes ferment dietary fiber. SCFAs help maintain gut barrier integrity, reduce systemic inflammation, and may support kidney health.
Listeners will learn:
• 🔄 How the gut–kidney axis creates a feedback loop between microbiome health and renal function
• 🧪 Why dysbiosis leads to toxin buildup and worsens CKD outcomes
• 🌱 The potential of dietary strategies, prebiotics, probiotics, and microbiome-targeted therapies to improve patient care
• 💡 Practical takeaways for supporting gut health in those living with kidney disease
This episode closes our gut axis series by connecting the science of the microbiome with real-world implications for kidney health, offering both hope and actionable insights for patients, providers, and caregivers.

In this fascinating PART 1 of 2 Episodes, I dive into the emerging science of the kidney–gut microbiome axis—a dynamic two‑way relationship that’s reshaping how we understand kidney health. The discussion explores how the gut microbiome influences kidney function through metabolites, immune signaling, and inflammation, while kidney disease itself alters the gut environment, creating a feedback loop that can either promote healing or accelerate harm.
Listeners will learn:
• 🔬 How gut‑derived metabolites like short‑chain fatty acids and uremic toxins impact renal physiology
• 🧬 Why dysbiosis (imbalanced gut bacteria) is linked to progression of chronic kidney disease
• 🍎 The role of diet, prebiotics, and probiotics in modulating the microbiome for kidney health
• 🩺 How this axis opens new doors for therapeutic strategies, from microbiome‑targeted nutrition to novel pharmacologic approaches
This chapter highlights the interconnectedness of systems—reminding us that the gut and kidneys don’t work in isolation, but in constant conversation. It’s a cutting‑edge look at how microbiome science is transforming nephrology and nutrition practice.
