Some moments change how you see medicine forever. Once you see them, you can't unsee them.

Hosted by pharmacist Leonard Pastrana, PharmD, this podcast goes inside the private conversations happening at the leading edge of clinical longevity medicine. It features physicians, scientists, and founders who are seeing things in their patients that the textbooks haven't caught up to yet.

Too often, "show me the data" is where the conversation ends. Here, it's where it begins. Each episode explores the gap between what we know for sure, what doctors see in practice every day, and the uncertainty in between. You'll hear how the best clinicians think, question, and make reasonable decisions when definitive proof doesn't exist yet.

Longevity medicine isn't a new set of rules. It's the evolution of medicine: improving how well you live and how long you live, at the same time.

If you've ever felt like there has to be a better way, these are the conversations for you.

Pharmacist and compounding pharmacy owner Dr. Franck Kacou joins Leonard Pastrana to break down one of the most misunderstood issues in peptide therapy: the difference between peptides compounded by a licensed pharmacy and products labeled "for research use only."

Franck has spent over a decade running a compounding pharmacy now licensed in multiple states, and he's watched peptides go from a physician-led niche to what he calls "the social media economy." In this conversation, he and Leonard get into:

• Research vs. compounded peptides, and why the raw ingredient makes all the difference
• Why research peptides are so cheap, and what that price is really telling you
• How physicians unknowingly buy and sell research peptides to their patients
• Why a certificate of analysis (C of A) doesn't tell you as much as you think
• How to verify you're actually buying from a licensed 503A or 503B pharmacy
• The GLP-1 shortage and how it changed the way physicians buy medications
• The FDA Pharmacy Compounding Advisory Committee peptide vote, and what it could mean for compounded peptides
• The growing push to bypass physicians with direct-to-consumer models

Franck also shares how burning out in traditional retail pharmacy led him to functional and longevity medicine, and the moment he couldn't unsee.

This episode is for educational purposes only and is not medical advice. An FDA advisory committee recommendation is not FDA approval. Talk to your licensed healthcare provider before starting any peptide or medication.

→ Why physicians are now small business owners who have to earn trust
→ What it means to "operationalize" a brand inside your practice
→ Why consistency feels boring and why it works
→ Word of mouth as the most powerful growth channel in healthcare
→ How you measure brand when there's no clean KPI (value, consistency, and authenticity signals)
→ Neuroaesthetics: how the brain and body respond to brand experiences
→ Phenomenology and why subjective experience still matters in data-driven medicine
→ How the room design at CALM changed how a skeptical physician received the content
→ Masterminds, community, and finding your tribe
→ Kelly's book, Breakout Creative

You can't fake a brand, at least not for long. Get clear on your North Star, surround yourself with people who share it, and the brand takes care of itself.

Some moments change how you see medicine forever. Once you see them, you can't unsee them.

For Dr. Haleem Mohammed, known as Dr. Hal, one of those moments happened at home. He was a chief medical officer managing thousands of employees, and he had reached 255 pounds. His wife, also a physician, asked him a question he couldn't ignore: would he be around after they had a baby?

He resigned, rebuilt how he lived, and rebuilt how he practiced. Today he's a board-certified internist, Chief Medical Officer of Gameday Men's Health, runs his own concierge practice, and serves on the FDA's Pharmacy Compounding Advisory Committee.

Leonard and Dr. Hal go from Wall Street to the FDA hearing room. They talk about how insurance shapes what doctors can and can't do, and why he tells some men they don't need testosterone. They cover what a network of hundreds of clinics reveals that a single practice can't, and what it was like to push back during a committee meeting on the future of compounded peptides.

In this episode:
-His path from stockbroker to chief medical officer
-The 255-pound wake-up call that changed his life and his medicine
-How insurance shapes what doctors are allowed to do
-The difference between a low testosterone number and testosterone deficiency
-His advice for doctors who want out of the insurance model
-What the FDA advisory committee actually does, and what it doesn't
-What drives him now

For Dr. Haleem Mohammed, known as Dr. Hal, one of those moments happened at home. He was a chief medical officer managing thousands of employees, and he had reached 255 pounds. His wife, also a physician, asked him a question he couldn't ignore: would he be around after they had a baby?

Some moments change how you see medicine forever. Once you see them, you can’t unsee them.

Dr. Kris Wusterhausen was doing everything he’d been taught. He was running a county clinic, he was burned out, his weight had climbed to 199 pounds, and his A1C had reached 6.2. Fixing his own health changed how he practiced medicine. In his words, it took him from a widely accepted family doctor to “the kooky doctor in town.” Years later, some of the doctors who called him kooky became his patients.

Today he’s a board-certified family physician and founder of The Resurge Clinic in Weatherford, Texas. He trains physicians in hormone and peptide therapy and serves on the FDA’s Pharmacy Compounding Advisory Committee.

Leonard and Dr. Kris go inside the committee meeting where BPC-157 and other peptides were reviewed. They cover what the committee was actually asked to decide, and why real-world clinical data deserves a seat at the table. Dr. Kris also shares the financial reality of leaving traditional medicine, his keep-it-simple approach to hormones and peptides, and why he brings his entire staff to medical conferences.

In this episode:
-The burnout and health crisis that pushed him out of traditional practice
-The financial reality of transitioning to a new practice model
-From “kooky doctor” to accepted: how the field has shifted
-Inside the FDA Pharmacy Compounding Advisory Committee’s peptide review
-Why the vote was about compounding safety, not FDA approval
-Why great clinicians rely on a handful of peptides, not twenty
-Nutrition, movement, and the 80/20 approach to longevity
-Why every person in a practice should understand patient care

<itunes:summary>Some moments change how you see medicine forever. Once you see them, you can't unsee them.
In this first episode Leonard Pastrana, Pharm D shares the "unseen moments" that reshaped his view of healthcare. They include a hospital committee that chose a treatment based on cost instead of what was best for the patient, and his wife's years of recurring diverticulitis flares that specialist after specialist couldn't explain. It took a physician willing to look beyond the guidelines to finally find what everyone else had missed.

After working in retail and hospital pharmacy, and then spending years creating education alongside leading physicians and scientists in clinical longevity medicine, he's had thousands of private conversations with doctors who practice in the gap between what's proven, what they see in patients every day, and what we're still figuring out.

This show brings those conversations into the open. You'll hear from physicians, scientists, and founders who treat "show me the data" as the start of the conversation, not the end.

In this episode:
- What an "unseen moment" is, and why it happens so often in healthcare
- Longevity medicine defined: improving health span and lifespan together
- Why earlier diagnostics are creating new uncertainty in medicine
- The personal story behind the show
